PROGRAMS & SERVICES

YSCC does not charge fees to the public for any service. Donations are always welcome.

YSCC is formed to carry on exclusively the charitable programs of Emergency Shelter, counseling, education and treatment of persons experiencing problems in society. The terms “charitable” and “education” shall be understood to have the meaning in a board civic and social welfare sense as defined in the Regulations issued by the Internal Revenue Service pursuant to Section 501 (c) (3) of the Internal Revenue Code and/or any successor to that section. YSCC is non-profit, non-sectarian, and non-political in all its policies and activities.

Intake & Assessment

Initial Intake Form
The information to be obtained on all applicants for referrals for admission include the nature of the presenting problem, client demographics, and referral sources to be recorded on the initial contact form.

Referral Procedures for Clients
An appointment will be made with the referred client and or legal guardian.

On appearance for services, an intake will be completed on the client which includes the legal consent of the parent or legal guardian.

An authorization “Consent to Release” will be obtained if any transfer of client information is involved.

The referred client will be accepted for services if all criteria are met and regular appointments will be scheduled.

During the intake process ask the child if they have been abused. If abuse is suspected the DHS State Office, Office of Client Advocacy and the OJA Office of Advocate General are to be notified.

Client Records
Client records will include but not limit to the following:
       1) Intake form
       2) Client status form
       3) Daily contact sheet
       4) Treatment plan
       5) Resident contract
       6) Shelter referral sheet (if applicable)
       7) Release form (If applicable)
       8) Consent for admission or participation
       9) Daily service log
       10) Assessment form
       11) Review of treatment plan
       12) copies of any correspondence pertaining to client.
       13) Consent form signed by parents or legal guardian
       14) Client’s rights form signed by client.
       15) Discharge summary to be completed within fifteen (15) days of case closing

Client Data to be Recorded During the Intake Process
The following is only a guideline:
First section
The Intake form needs to be as complete as possible. Make every effort to get all the information requested on the intake sheet.
Second section
       Confirmation of contact of parents or legal guardian is to be included in file. This is included on the parent consent form and on the admitting form used in the Shelter.
       Person requesting placement.
       Other agency contact person and home telephone numbers if applicable.

       The parent and/or legal guardian or admitting party are to sign the admission
responsibility form.
       If a judge issues a verbal detaining order, the employee of YSCC is to write down this information, including the date and time of day.
Third Section (Shelter only)
       All medical background information will be obtained from the parent or legal guardian to ensure YSCC has all pertinent information for the child’s safety.
       The parent or legal guardian are to sign the medical responsibility form.
       The parent or legal guardian must sign a medical consent form for YSCC to administer any medications, prescriptions or over the counter drugs.
Fourth Section (Shelter only)
       The employee of YSCC doing the intake must obtain a list from the admitting party as to who will be permitted to be on the telephone list and visitation list. Also if the client is to be permitted to participate in activities sponsored by YSCC.
       A complete inventory is to be made on the belongings of the child, including but not limited to; clothing, personal items, money, etc.
       Information will be obtained regarding school arrangements: who is responsible to get the child enrolled in school, transportation etc.
Fifth Section (Shelter only)
       Documentation is to be made of all telephone calls made and received.
       No long distance telephone calls are to be made by the child and charged to YSCC.
       Telephone calls made to the child by people not on the approved list will not be accepted.
Sixth Section (Shelter only)
       If the child has the ability to read, let them read the rules.
       If the child is unable to read, have the rules read to them.
       After the rules have been read, the employee of YSCC and the child are to sign the rules form and have the understanding these rules are to be followed.
Seventh Section
       copy of the rules and regulations for residents of YSCC will be posted in the Shelter. Also, the discipline plan will be posted.
Eighth Section
       The employee of YSCC will have the child and/or the parent or legal guardian read the Grievance Procedures.
       If the child and/or parent or legal guardian are unable to read, the employee of YSCC will read the Grievance Procedure to the child and parent or legal guardian.
       The grievance form is to be signed and dated by the child and the employee of YSCC.

SERVICE
The following guidelines are to be observed in an effort to protect the confidentiality of all people either applying for, or receiving services from YSCC.
       Communications regarding clients or client activities are not permitted outside the agency. Individual clients are never to be discussed by name outside the agency.
       Clients and/or client-related issues are not to be discussed in reception areas or where others are present to overhear the conversation.
       Do not leave information (letters, appointment books, or notes) which contain client names where it could be read by anyone. At the end of the working day, all information containing a client name should be locked up.
       If you meet a client away from the agency, wait for the client to speak first. This method of protecting the privacy of the client is to be explained to the client at the time of application.
       If you know a client personally outside the agency, do not initiate remarks regarding services being rendered at the agency. Discourage discussion about the individual’s services at YSCC when the client initiates the topic.
       Every effort must be made to protect the privacy of a client. A breach of confidentiality is grounds for dismissal from employment at YSCC. If a breach in confidentiality occurs by a non-staff member which results in legal action against themselves, they will indemnify and hold harmless YSCC.
       EVERYTHING IS CONFIDENTIAL

Treatment & Follow-Up
       Follow-up shall be conducted on all clients as required by funding agencies annually after discharge from the services provided by YSCC.
       Information from client follow-up shall be incorporated as a part of the program evaluation.

Referrals
YSCC shall refer clients to other licensed or certified facilities or resources when the individual has treatment or service needs that the agency cannot meet. In addition, the program shall provide information and referral services to non-client individuals who request these services.

Program staff and clients shall discuss the purpose and nature of the proposed referral before implementing the referral. Family and significant other(s) will be involved when appropriate.
